Vintage Bausch & Lomb Vertometer, catalog number 21-65-70 with instruction manual.  Per AI via 20/20 magazine:  “A vintage Bausch & Lomb Vertometer (commonly known as a  lensometer or focimeter) is a precision optical instrument used by optometrists, ophthalmologists, and opticians to measure and verify the exact prescription and optical properties of eyeglass lenses.”   Top section is 21″ wide, and the base is 7″ deep in the front.  Quite heavy.
